 Frederick Wiseman’s debut feature, Titicut Follies, plunges unflinchingly into the grim realities of the Bridgewater State Hospital for the criminally insane in Massachusetts. Famously banned from public exhibition by court order for decades—and shown only to medical professionals until 1991—it remains one of the most controversial and essential documentaries in American cinema. Wiseman’s camera exposes inmates being stripped naked, taunted, force-fed, and locked alone in bare cells. We witness, almost in real time, the casual, soul-crushing indifference meted out to men with varying degrees of mental illness. 

Shot with available light and unvarnished sound, the film’s raw aesthetic only heightens its intensity. Among its most unforgettable sequences is a scene in which ward staff “clean” a patient’s room, using the very process as a form of torment. The title refers to the hospital’s annual variety show, staged by staff and inmates, which Wiseman uses as a framing device. These performances, incongruously cheerful amid the surrounding cruelty, reflect the director’s career-long fascination with theater as both spectacle and metaphor.
